Diabetes – annual diabetes reviews and management

Our nurses are available for your annual diabetic review and follow up visits as required to help you develop a management plan for living with and reducing the risks of Diabetes. We can link you in with fully funded services including the Desmond Group education programme, or individual sessions with a Health Coach or Health Improvement Practitioner (HIP) as appropriate to help you gain the tools required for successful management of diabetes.
