GORE MEDICAL CENTRE PATIENT CHARGES

The Gore Medical Centre is part of WellSouth, the Southern Primary Health Organisation (PHO) which provides some subsidised funding for patients who are enrolled with it. For patients who attend and are not enrolled, there is a higher fee, however, this is reduced if the patient has a Community Services Card (CSC).

The fees listed below are for a standard 15 minute appointment.

A fee of $15 applies to all referrals.

* Repeat prescriptions need to be ordered well in advance. Please allow at least 48 hours for it to be ready; and longer over weekends and public holidays. If you require an urgent repeat prescription (on the day), there will be an extra charge.

Childhood schedule immunisation – no charge

Medical Certificates (eg driving, diving) are not part of the PHO funding and incur a full non PHO fee.

Home visits by arrangement only.

Other extra examinations may incur an extra fee. Please ask at reception.

Additional fees not listed above may apply as appropriate. Charges apply for advice provided by a GP or nurse by telephone.

Payment of fees is expected at the time of your consultation.

Gore Medical Centre terms of trade:

  1. Prices include GST unless otherwise stated.
  2. Unless otherwise agreed, all services shall be paid for on the date of service. Accepted forms of payment are cash, EFTPOS, Visa/Mastercard, internet or telephone banking or automatic payment. Please note a surcharge applies for credit card payment.
  3. If pay,ment is not made on the day of service, it must be paid within seven days of receipt of invoice.
  4. The Gore Medical Centre may withhold further provision of services where there is any outstanding amount due.
  5. Patients who are in breach of the agreed payment terms will be referred to debt collection and/or legal proceedings. Costs incurred to recover outstanding monies will be charged to the customer.
  6. Non-attendance at GP appointments will incur a fee of $25 for adults ($10 for CSC holders). Non-attendance at nurse appointmnets will incur a fee of $10 ($5 for CSC holders).
  7. Charges apply for advice provided by a GP or nurse by telephone.
  8. In the event an ACC claim is declined, a standard consultation fee will be charged.
  9. Cancelled appointments on the day may incur a fee.

Overdue accounts

If all or part of your account has been overdue for 60 days or more, unless it is a medical emergency, we will ask you to pay for any service prior to seeing the Doctor or nurse. No further credit will be granted until your debt is paid in full or you have established an automatic payment plan that reduces your debt.

The Gore Medical Centre reserves the right to decline further non urgent treatment where outstanding debts are not addressed.
